Record sheet · one page, twelve rows
Shipment gate record
Print this page. Fill it at the moment of judgement, not afterwards. One sheet per order — a sheet covering several orders records none of them.

Evidence law, applied first: revision □ two signatures □ traceable number □ — any box empty makes the verdict Unclear, not Pass.
Every Unclear must leave with a name and a date: ______________________ / ____________
Release signature: ______________________ QA counter-signature: ______________________
01How to fill it without fooling yourself
Three rules that decide whether this page becomes evidence or decoration.
- Write the reference, not the opinion. “Report R-2418, rev A/0, lot 240915-3” is evidence. “Looks OK” is a mood.
- Tick one box only. If you want to tick two, the answer is Unclear.
- Sign your own line. A sheet signed by one person for all twelve is a self-statement, and it is the first thing an auditor discounts.
Keep the sheets. Twelve filled sheets from twelve consecutive orders tell you more about a supplier than any questionnaire — and after one quarter they show exactly which check fails first.
